- You may contact our clinic at (919) 660-2100 to schedule an appointment or contact one of our affiliate Health Centers directly by telephone. Our department's full appointment information can be found here.

this page under the heading "Health Center Appointment Information."
You now have the option of making an appoint with our department online! Simply fill out the appropriate form and our appointment staff will contact to confirm your request.
- If you are a patient, you may obtain an appointment by completing the online Patient Self-Referral Form.
- Physicians may refer a patient by completing the online Physician Referral Form.
